Well It looks like Fatty liver especially with the seeming insulin resistance going on. A 5000kcal Bulk, GH and gear will definitely push your glucose up.
Metformin will help with the insulin resistance. Fatty liver should reduce with reduced steroids, reduced GH or with the addition of Reta.
Reta is not a quick fix for fatty liver though

Reta better then tirza?

Thoughd tirza is better for insulin resistance

For Fatty Liver Reta is better. For Insulin resistance Tirz is better, but since the plan involves taking metformin, then metformin and reta. If the goal less rapid but longer lasting inulin resistance control as well as fatty liver resolution, then just tirz is okay (will normalize after some months). Nothing wrong with taking Tirz and metformin though.. but it's gonna be hard getting to 5000kcal on Tirz, except if you are used to it.

Reta is king for NAFLD
Old graphs, i don't have anything newer since i don't really keep up to date with GLPs. I think 15mg of Tirz gets you a 50% something reduction by week 52? (Don't quote me on that, do your own research lol)

You could always go down to a baseline diet for 2-3 months then retest to see where you are at. As opposed to throwing in more drugs that may just muddy the water. Depends on how long term you are looking when dealing with your health. I had high liver enzyme levels for over 25 years from eating a lot of protein and training hard. Once those things were lowered levels came in normal and no fatty liver.
