HCG question

2 weeks in to my HCG being added to my TRT and I have not noticed any increase in testicle size. I am currently taking 250iu 2 times a week the same day I take my test. Have I not given it enough time yet? I read a few threads where guys were feeling changes almost immediately. Any insight would be appreciated.

How long were you on TRT before you started HCG? Did you notice any substantial shrinkage when you got on TRT?

If I do a large dose of HCG I can notice a very subtle dull ache in my testes sometimes.

about 4 months before I started HCG. Definitely lost some size. Even my wife noticed. It's not horrible but I don't want it to get worse.

Perhaps 250 HGH will not be enough for you, since the dose is sometimes very individual and depends on the situation.
How long would you recommend that I wait before I increase the dose? Also, by increase i will probably space it out over 3 injections as opposed to the 2 i do now.

A few points for the OP:

1. It might take a lot longer than 2 weeks to see an increase in testicular size, no matter how much HCG you take. TRT can really suppress testicle size. You didn't mention your T dose/frequency, btw. The more you're taking, the more suppressive it is.

2. Any time you change your protocol - adding in HCG with T is a change in protocol - you need to get labs done (minimum: Total T and estradiol) after 4 or 5 weeks on the new protocol. Why? HCG can increase your estradiol and de-rail the effectiveness of TRT. Do this before making any changes to your dose/frequency of HCG.

3. Using HCG with T is, generally, not very easy to get to work properly. Your goal is to find the minimum dose of HCG you need to get some/all of your testicular size back. Given how suppressive TRT can be to testicular function, you might never find the right dose/frequency of HCG.

4. Consider going on HCG only and add in HMG once you have it dialed in for a good T level while minimizing the estradiol response. Yeah, I know, HMG is expensive.

Does HMG increase testosterone? I thought it only increased sperm production?

I am also interested, if anyone knows and can describe the mechanism in detail, I will say thank you very much for the explanations.

It doesn't increase T, but it can increase testicular volume. When the Sertoli cells come back online due to adding in HMG, the testicles can increase in size.

Gotcha.
You mentioned years ago that hCG raises estradiol in 3 ways. One was from aromatization as a result of increased testosterone levels, one was from aromatization due to increased intra testosterone levels and the 3rd way I cannot remember. What was it again please?

Two ways:

Conversion of T to E2 in the testes via adipose tissue (aromatase) around Leydig cells, then

Peripheral conversion of T to E2 when the T from the testicles gets out into circulation, again via adipose tissue (aromatase).
