It happens often in cold weather or post by airplane
the way most of us use vials is we draw some air into a syringe and then push it inside the vial before we draw the liquid. Sure i agree air can carry bacteria that contaminate things but its probably fine here. Especially if the vial only cracked recently.
Looks like the steroid API has fallen out of solution and crystalized. I used to get that a lot with Test Cyp home brewing. I stopped brewing it, it was that common as I liked to make higher mg per ml. Test E and Test U are much easier to brew IMO so switched to them.
You can often salvage it by putting it into a warm cup of water and it may go back into solution (This only really works if its crashed due to living in a really cold climate)
If its not due to cold weather, chances are the bottle is higher dosed then normal. It can happen when brewing if you don't keep the liquid moving during manual bottling, you can get settling and the last bottles will have higher dosage per ml and might not hold as well in solution. Other option is not enough solvent used.

Absolutely. Like I said, I'm not an expert and my exposure to test cyp dynamics is limited compared to a lot of the folks (and especially brewers) here.
I have been led to believe that the standard recipes of e.g. 2% BA / 20% BB, when kept at at room temperature and 1 atmosphere pressure, aren't too likely to crash for concentrations of 250mg/mL and under. But I can't find any test cyp raws to practice brewing with so I can only go by what I read and my personal experience as a buyer of finished products
