On that note, given the side effect profiles of each(low as they are), if a lower dose combo of Telm/Ciln doesn't bring BP into range, I think it's preferable to raise the Ciln instead of Telm. There are more kidney protective and other benefits from the higher Ciln dose, while raising Telm slightly raises the risk of excess potassium buildup. I only mention this because usually the ARB would be raised first when more BP reduction is needed.
Think generally you’d be right, but gets a little bit dicey if someone is running heavy AAS. The more AT blockade you can have for the antifibrotic cardiac properties, the better.

For someone running moderate doses/cruising, would totally agree.
 
I was hoping to start at 20mg Telmisartan, but they only have the 40mg (TELMAHEAL 40MG 1X15 TELMISARTAN) available. Does anyone know if these are scored or otherwise able to be split into two doses?

I can't remember if they're scored, but I got a pill cutter off Amazon for this kind of thing. I know that everyone is different, but I will say I didn't see a change until I started taking 40 mg a day.

40mg is the standard starting dose for just about everyone. Good balance of sides and results at that dose. You can like stated get a pill cutter and be OK, but personally I would just do the 40mg and make it easy.

Thank you for the responses. I'm struggling to control my BP due to having hyperkalemia (high potassium blood levels) and being unable to take ARB's and ACE inhibitors as they all have the potential to raise potassium levels. I was hoping to start low, at 20mg, and get blood work done at that dose, then again at 40mg is my potassium was okay at 20, but my BP was still high. Combining it with Cilnidipine.

My concern is that I've read that non scored tabs are not necessarily homogeneous in active drug distribution throughout the tablet, and cutting one on half will not yield 50% dose per piece. It's better than nothing, so I'll still give it a try.

Also, I was just kindly messaged and reminded that telmisartan has a coating, and it's not advisable to cut coated tabs. I think the listing for the particular version of telmisartan I'm getting says not to cut them. Oh well. Hopefully I can do okay with 40mg

It's worth noting if you're at 80mg Telm, bumping Ciln from 10 to 20 at that point only lowers systolic and diastolic by another 2-4 points each, but, by increasing Ciln, you not only increase kidney protection by reducing pressure inside the kidney, studies show Ciln, especially 20mg, improves potassium handling when used alongside Telm, reducing the risk of telm induced hyperkylemia.

A perfect combination.

Cilnidipine improving potassium handling and helping to prevent telmisartan induced hyperkalemia is the best news I've heard in a long time. Here's hoping it's enough to keep my potassium in range

If you’re doing it for purely lowering BP a lil bit, protecting kidneys, and improving potassium handling, why not layer in a small does of empaglifozin?

Lowers BP 3-4mmHg, lowers hyperkalemia even on patients with CKD on ACE/ARB, protects kidneys, improves ejection fraction/lowers cardiovascular mortality.

I’m like 60% kidding/mostly busting your balls and just having fun keeping up. All playing at the margins here.
