I know the bitcoin ATM i use doesn't ask for anything other than a phone number. Just got to be careful of the fees though

I think the LibertyX app will let you pay cash at convenience stores if there is a participating one near you.
They have a limit though, maybe $200 a day?

Localbitcoin or atm cash just buy prepaid SIM card in cash or in some anonymous way so you can then withdraw bitcoin from ATM anonymously. Cover your face from the camera as well

To get a wallet you can do it in may way anonymously! To buy bitcoin it's a different story, online mostly you need to provide documents etc. if you use ATM bitcoin just phone number if you use localbitcoin you need to provide nothing only cash to the other person.
