What to stack on top of Tirzepatide for food noise?

I'm on Tirz 10mg, appetite suppresion is no longer enough tho, especially the "food noise" gets really bad. Like I could down large plates of lean meat and vegetables and then go the whole day fantasizing about food. the Tirz nuked my appetite and food drive at first but as I lost weight it came back (as expected).
I don't want to up the Tirz to 15mg because I think my GLP1 and GIP receptors are fully saturated or close to. because 5mg at first made a drastic difference, then going from 5mg to 10mg gave only a little extra appetite suppression, barely noticable.
So what else could I stack that suppresses appetite trough a different pathway? I was thinking either metformin (for leptin receptor agonism) or Cagrilintide? Anyone has any suggestions?

Tirz had very strong appetite suppression for me at 10mg. I've used cagrilintide with Reta and it worked okay. Semaglutide is supposed to have more appetite suppression than tirz - switching might be worth a shot.
 
Your receptors aren't saturated. You're describing how Tirz, and all GLPs work. Yes, it's "diminishing returns" in terms of dose volume, but it's very rare for someone to not respond to 15mg after being on 10mg. In fact we know response continues for almost everyone to 25mg based on the current clinical trials for the new larger doses Eli Lily plans.

That said, a VERY small dose of Sema will turbocharge appetite suppression (and potentially sides in proportion).

Even if you know what .25mg Sema felt like before, and it was weak or ineffective, don't assume it'll be like that stacked with Tirz. It can be extreme. So be very careful. Add .25mg Sema / week, on any day, it doesn't have to be the same day as Tirz. And wait 2 days before deciding if it's effective or not.

trying sema makes sense aswell because he might just be a "non responder" to gip while being a strong responder to glp-1 or the other way around.

15mg sounds insane to me im a hyper responder, 2mg take it or higher and cant eat

He's a responder, he said so himself. He just didnt realize 10mg wasn't the huge dose he though it was.

Go up to 15 and enjoy the continued suppression.
 
I'd suggest continuing up to 15 mg and if / when that stops working in terms of appetite suppression, consider adding Cag.

But can you say more about your caloric intake / output, weight loss history, other supplements, etc? Are you doing a lot of cardio? When I first started Tirz I had to add in more cardio just to get my appetite up enough to get 2000 calories in (I was 245 when I started).
 
im a former fat fuck, i went from 440 pounds to currently 260 making me still a fat fuck just not a whale fat fuck lol and still climbing down. i stack the tirz with some cagri and it works wonders.

That sounds counter intuitive
Cardio always stimulates my appetite. I keep it in when bulking for that reason. I usually just aim for a step count when cutting and then add in a step mill if I have stubborn areas that need to come off near the end. But the Tirz was a sledge hammer to my appetite and I needed to add in more LISS to get my appetite up enough to get in the bare minimum of protein at least. That's worn off as the weeks have gone by but for the first 48 hours after each shot (i split it into twice per week) the appetite suppression is very strong.
 
Im in exactly the same situation as OP. I was running 10mg tirz and could see in my food/weight log that I was not getting the same level of outcome from 10mg as before.

What did I do? I jumped straight to 15mg and now Im back to where I want to be in terms of appetite suppression.

Im also now at the point where I will escalate beyond 15mg if I have to. I´ve used these drugs long enough now to know that I do not experience any terrible sides. Even jumping from 10 to 15mg, the only thing I have to watch for is the pace at which I eat. If I eat the first spoons of rice too quickly I get instant indigestion which passes after 2 minutes.

for me digestion-wise and appetite-wise I feel like I'm the same as before Tirz now. I have lost a good amount of weight so not complaining but miss that lack of food drive and being full after a regular size meal that Tirz initially gave me. Now I'm back to having to resort to eating large amounts of low calorie foods and intermitting fasting to not overeat (not saying Tirzepatide lost effect, it did what it was supposed to do as it brough down my "weight setpoint", just I miss that feeling of not constantly thinking about food and having to worry about overeating)

I'm around 2600-3000cals rn (varies, I don't track strictly but I know my TDEE is a bit above 3000cals, I usually can gain or loose weight just by eyeballing my food, issue is overeating lol). I lost weight many times naturally. I'd keep it off for maybe 6ish months and then regain it, so I decided to go with GLPs this time around. I'm almost at my goal, I want to loose another 6-8lbs of bodyfat.
I do a little cardio, should do more but I have very little energy recently. I'm looking to fix that as well so I can do a proper amount of cardio (I'm probably thyroid deficient. waiting to get blood work to confirm).
At first I also could barely eat 2k cals, but now I still hungry on 3k cals (I lost about 30lbs since starting Tirz)

Macro breakdown? I had to lower protein and increase carbs (rice) to keep my appetite going. Maybe you need to increase protein / fats?
 
around 200g protein per day. I don't count carbs and fats but I eat a lot of eggs, avocados, olives and walnuts so I get a good amount of fats
Sounds like plenty of fats. Might help to track carbs and when you get a sense of the calories from carbs swap out 50 g carbs for another 50g protein. Perhaps that can help with satiety.

I just added cagri last week at .25mg and it seemed to help. But what I realized is that on 500mg test or more I can eat through tirz. I was at 20mg UGL tirz and I would say the food noise sorta comes and go. It has slowed down a bit more since adding cagri but I also dropped down to 15mg tirz to see if I could cut back on it. If this works I’m going to step the tirz down to 12.5 then to 10 and slowly bring up cagri if it is needed.
