What happened to my vial

I had a reconstituted vial of HCG that I was maybe half through develop a weird crust on top. I tossed this but what would have caused it/what should I have done differently? This isn’t my first peptide but I’m no expert

For reference this was a 5000iu vial of HCG reconstituted with 1ml BAC using a 23 gauge needle - I almost wonder if the needle was too big as this seems to have seeped out of the middle of the stopper

Wipe it off with alcohol swabs Looks like some just spilled and dried up.

Most likely dried up residue use alcohol wipes
