What concentration are you aiming for, and what amounts of TP, benzyl alcohol, benzyl benzoate, and oil did you use?
The reason for crystals coming out when cooling is always because of using more steroid than the vehicle can in fact hold at room temperature.
EDIT: Well, where there is nothing but steroid in the powder. But usually the problem is not foreign material.

It's super saturated so you can either increase the volume until it stops precipitating or increase the temp to see if the saturation level enough to dissolve the crystals.
From your previous experience with the same solution, what you have is supersaturated (more steroid is presently dissolved than in fact can be held long-term.)
It simply hasn't crystallized out YET.
Being supersaturated, it will crystallize sooner or later, whether in the vial or possibly in your body.
You don't have the figures for how much TP, BA, BB, and oil?

After reheated you will need to add more oil and solvent...you'll need to figure the amount and what your dosage will be after. I've used this method on a small scale...not sure what you have going though.
Did you try and make the mg/ml ratio too high maybe?
